PATIENT INFORMATION

Payment &
cost estimates.

Understand the visit fee and ask about expected costs before choosing your care.

Direct-pay care.

Vitalhaus does not bill insurance. Your visit fee and what it includes will be available to review before scheduling. For concierge memberships, the recurring fee, included services, renewal and cancellation terms will be available before enrollment.

Your visit fee includes review of relevant prior lab results, test reports and clinical notes you provide before your appointment, along with provider time, care planning and education within the scope of your visit.

The cost of new lab tests, imaging, medications, supplements or care from other providers is separate from your Vitalhaus visit fee unless specifically included. If a separate follow-up appointment is needed, its fee will be explained before scheduling.

PATIENT RIGHTS · SELF-PAY CARE

Your right to a
Good Faith Estimate.

If you do not have health insurance or will not use it for your care, you have the right to a written Good Faith Estimate of expected charges for non-emergency health care. You can request one before deciding whether to schedule.

Your estimate describes the care planned and its expected cost, including related items and services reasonably expected as part of that care. It is separate from this website’s general pricing information.

When you receive it

When you ask before booking: within 3 business days of your request.

When you book 3–9 business days ahead: within 1 business day after scheduling.

When you book 10 or more business days ahead: within 3 business days after scheduling.

Keep it with your records

You can receive your estimate on paper or electronically. Keep a copy to compare with any bill you receive.

For separately scheduled testing or care, request an estimate from that provider or facility too. Receiving an estimate does not commit you to care.

Please keep medical details out of ordinary email. We can provide secure instructions for any information needed.

Your billing rights

You may use the federal patient-provider dispute process if a provider or facility bills you at least $400 more than its Good Faith Estimate and your bill meets the program’s eligibility requirements. Start the process within 120 calendar days of the date on the initial bill.

For details about these rights, contact CMS at 1-800-985-3059 or visit the CMS billing-rights guide ↗.
